PRICES (Paid at time of service)

  • We are In-Network with several Washington vision insurance plans including: Aetna, Spectera (a specific vision plan sometimes offered through United Healthcare), EyeMed* (Costco Employees only), Regence, Blue Cross/Blue Shield, Premera** & Lifewise of WA. ​​

  • Please consult with your specific vision insurance plan and then we are happy to verify your coverage with Woodinville Family Eyecare before your eye exam.

  • Coverage varies per your specific plan with your insurance company.
  • Please keep in mind that patients are responsible for any balance due not covered by insurance, even after billed. All eye exams/office visits/post ops/other services billed are subject to your specific contract with the insurance company on the date of service.

  • We are OUT of Network (we cannot bill for you) for the following vision insurance plans:
  • VSP (Vision Service Plan), EyeMed (except Costco employees), MetLife, Davis, Cigna, Kaiser, Molina, Apple, Ambetter, First Choice, Medicare, Medicaid, Premera Med-Advantage, United Healthcare (without Spectera vision plan) & others not listed above. 

  • For these and other insurance plans we do not bill, you will need topay at time-of-service for your eye exam (see prices above). We are happy to provide you with two detailed receipts to send in for possible reimbursement. Check with your insurance to find out if you can receive reimbursement if you have an eye exam with Woodinville Family Eyecare, Dr Kevin Y Hirano, OD. Please note for insurance coverage purposes: We are a separate, private Optometry practice (Eye exams/Optometrist), independent of Costco Optical (hardware: eyeglasses and contact lenses for purchase). Therefore, each business might have different insurances we bill. 
  • ​*EyeMed is billed for COSTCO EMPLOYEES ONLY.
  • ** As of 2025, the Microsoft employee vision benefit is now covered through EyeMed. Unfortunately we cannot bill on your behalf, however, consult your plan regarding coverage if you are still interested in seeing Dr Hirano, Dr Zheng or Dr Tan. We have many previous Microsoft patients who would like to continue vision care with us. We are happy to provide you with an extra receipt for possible reimbursement.
